When Cox says this, they mean that they don't charge extra for an HDTV cablebox (if you don't have an HDTV they have a different cable box) and you get the HD versions of a growing number of many of the major cable networks on their Expanded Basic Digital (i.e. 500 channels of crap) - there was probably about 40 last I had Expanded Basic Digital
On the Limited Basic (i.e. $13 a month) you have no Cox box, but any newer HDTV has an internal tuner that should be able to scan and tune in the HD versions of many analog stations you're paying for - again no additional charge - assuming your TV has a tuner.
The cable plugs in directly to the HDTV with no box or HDMI - the quality is as top notch as you would expect.
