The model number of the individual units really helps, but I'll make some assumptions and go from there.
My suggestion on hooking up the various components (once you follow Bigloww advice on the handshake issue):
OPTION 1
Cable box to AVR via HDMI (picture and sound)

Blu-Ray player to AVR via HDMI (picture and surround sound)

BR to AVR via optical (stereo sound-when you want to listen to a music CD w/o
the TV on-some AVRs will not play sound through HDMI w/o a complete
chain <handshake> others have a "monitor out" or off switch)

AVR to HDTV via HDMI (picture and sound to TV-You will need to buy one more HDMI cable-- 3 needed total
This means whenever you want to watch TV, the AVR must be on. That's OK for movies and such, but who needs the news in surround sound? I hooked my system up differently for the cable box and have the BR player going through the AVR as above.
OPTION 2
Cable box to HDTV via HDMI skipping AVR (my wife likes this because she only needs one remote (cable box's) to turn on/off & change channels to watch TV)

Cable box to AVR via optical (sound through the surround system for TV movies)
